How laser therapy rejuvenates

All lasers are not the same when it comes to skin resurfacing. When San Diego plastic surgeon Dr. D. Glynn Bolitho decided to bring in Smart Xide Dot Laser therapy to his practice, he performed his due diligence in terms of researching the science behind it before employing it in his practice.

Since its arrival, Dr. Bolitho’s aesthetic nurse specialist, Elizabeth Reed, has also performed this nonsurgical laser treatment on patients for skin resurfacing goals.

“I’ve had patients call and tell me they don’t want to do a super comprehensive treatment, but they want to get the benefits of the laser. So my suggestions are to split it up into two lighter sessions so they can get some nice results. We then manage the second session based on the results of the first,” she said. “It’s really great that we are able to tailor treatment to each person.”

Following laser therapy, Reed has her patients adhere to a skincare regimen in terms of an ointment and post-procedure moisturizer.

While Reed performs this procedure in her aesthetic treatment room, she also wants patients to know how the laser is also being brought into surgery. Dr. Bolitho is utilizing this advanced technology for facial rejuvenation procedures, such as facelifts.

“Dr. Bolitho is an amazing plastic surgeon and this laser is just another tool for him to employ,” she said.

While surgical facial rejuvenation can lift, tighten, and add youthful volume, laser technology can indeed resurface the skin.

“We are able to combine this so the recovery is all at once,” Reed said.

For those who are not undergoing facial surgery, Reed still does recommend a consultation so she can listen to a patient’s aesthetic goals and expectations prior to the treatment while assessing their candidacy.

Reed said an in-office treatment generally takes about two hours. First a topical numbing agent is applied to the skin, and when the skin is ready, the treatment length is 20 to 30 minutes. Following this, icing takes place for a short period of time.

“While the numbing cream prepares the skin for treatment, patients describe the therapy like a rubber band snap with heat behind it,” she said. “Your face feels a little bit warm which is why we do some cold compresses afterwards.

As far as the best times for laser treatment, Reed recommends the fall, winter, and early spring months. It’s suggested to avoid the summer months when people are retreating outdoors.

A laser treatment unleashes new fresh skin so it’s important to avoid any risk of sunburn. Reed also ensures that her patients have good solar protection instructions.

Facial rejuvenation after weight loss

As many can attest to, making the decision to lose weight takes patience and dedication. Reaching those weight loss goals is truly a celebratory milestone.

For those who have attained their goal by losing 20 to 25 pounds, while they have redefined their silhouette, they may also notice that their face has lost volume.

“Anytime you lose a significant amount of weight, there is a concern about volume loss and perhaps the change of laxity in your skin,” said Elizabeth Reed, an aesthetic nurse specialist who works with San Diego plastic surgeon Dr. D. Glynn Bolitho. “So what we want to do first is to restore that volume.”

Reed mentioned the optimal products she uses are Juvederm and Voluma.

While Juvederm is used to lessen the appearance of nasolabial folds and marionette lines, Voluma addresses the mid-portion of the face. According to Reed, beginning rejuvenation at the mid-face is a great starting point.

This usually begins with using one syringe of Voluma. Generally, Reed likes to deliver the Voluma first and wait a couple of weeks for the product to settle before addressing nasolabial folds. In some cases, this area may resolve itself due to the Voluma, or a patient may need a lesser amount of Juvederm.

The Cosmetic Injection Progression

One of the leading manufacturers of cosmetic injections is Allergan which has undoubtedly been described as the pioneer in a special niche of nonsurgical facial rejuvenation. Its best known product is Botox, a purified substance of botulinum toxin A. For years, Botox has become the “go to” minimally invasive treatment to smooth out “dynamic wrinkles” along the forehead, between the brows and around the eyes to treat crow’s feet.

Dynamic wrinkles emerge from repeated muscle movement and Botox helps by intercepting these movements to smooth out those fine lines.

Kybella, another injectable by Allergan that was FDA approved last year, is being considered as another pioneer treatment. It has made strides in eliminating the appearance of submental fullness, otherwise known to many as the dreaded double chin.

The main ingredient of this newest product is a synthetic form of deoxycholic acid. It works like a “bile acid” that can dissolve fat. When injected, Kybella has the ability to target and disintegrate this unwanted fat.

While each patient is different regarding their aesthetic needs, Kybella is performed in a treatment series. Before the injection, the area is mapped out.

Generally, patients require two to four treatments, and it is recommended to space the treatments apart every couple of months. Number of visits is dependent on the scope and thickness of the area being treated. The reason for this duration is to give Kybella the appropriate time needed to resolve the issues of submental fullness. At the next session, updated aesthetic decisions can be made.

The treatment takes under 30 minutes and patients may experience some swelling, redness, or tenderness for a short period of time. Icing is recommended to minimize any side effects.

According to media coverage, such as in the Quartz publication, “…the company [Allergan] is testing the drug for FDA-approved use on other paranoia-inducing rolls, including what some refer to as ‘bra overhang.’”

And yes, there appears to be other discussion treatment items on the horizon.

In the Quartz publication, it was noted that Amy Lewis, a clinical assistant professor of dermatology at Yale School of Medicine was interviewed by Allure Magazine.

“There have been attempts to dissolve fat before, but this is the first time it’s worked—and that’s pretty exciting. In the future, Kybella could be used to improve the appearance of stubborn fat pads elsewhere on the body, like bags under the eyes or so-called ‘bra bulges.'”

Research continues on whether or not Kybella can be used in other areas such as the following:

  • Inner knee fat
  • Thick ankles
  • Armpit overhang

Even though the summer months are coming to a close, sunscreen should still be on hand every day of the year. While many won’t be headed to the beach or poolside like they did during the lazy days of summer, the fact of the matter is when one is outdoors, sunscreen must still be utilized. And this includes overcast days.

Cloudy days can certainly mislead people into thinking that sunscreen isn’t necessary. And that couldn’t be further from the truth.

According to the American Academy of Dermatologists, nearly 80 percent of UV rays will penetrate through the clouds. The two types of UV rays which cause damage to the skin are UVA and UVB.

UVA rays pierce through glass and clouds and are the culprit of negatively impacting the dermis portion of the skin. Without solar protection, premature skin aging such as fine lines, wrinkles, and sunspots will appear. Too much UVA exposure has also been known to compromise the immune system.

On the other hand, UVB rays cause immediate damage to the skin on its superficial layers and has a direct influence in developing skin cancer.

While sunscreen messages continue to swirl in the media, new cases of skin cancer still emerge. Ways to connect people and the sunscreen message is a continual goal. With this in mind, a research team hailing from the University of Surrey had an idea they pursued.

“They examined the way sun-safe messages are conveyed to young women to see what, if anything, might change their sun-worshipping ways. They found that if women could literally see the damage the sun does to their skin via technology that ages them — that’s what has the most impact,” reported Carleen Wild of LifeZette.

When the results of the study were revealed, Dr. Fosko of the Mayo Clinic in Florida, who serves as the chair of the dermatology department, shared his viewpoint. He’s clearly saying, “…a picture is worth a thousand words.”

He went on to share that when younger people are delivered a message that is punctuated with personalized relevancy, the impact is greater.

Wild reported, “Researchers found that after seeing their own face prematurely sun-aged through the use of technology, young women took two times the number of free sunscreen samples and three times the number of skin cancer leaflets, compared to those women who had read text information about sun damage. They also showed a 30-percent lower belief in the skin’s ability to heal.”

While this study was published in Cogent Psychology, it affords those in the medical profession another level of education in conveying the sunscreen message to their patients – an absolutely invaluable piece of information that will help promote skin care health.
