Plastic Surgery Consultation: Plan Ahead

For some individuals, a new year means new possibilities. One pathway of possibilities may be aesthetics by way of plastic surgery. When one has located a board-certified plastic surgeon, the next step is scheduling a plastic surgery consultation.

It’s completely normal for a patient to feel a blend of excitement and nervousness at this appointment. Nevertheless, there are some things that a patient can do ahead of time to make the most of their consultation.

Karen Martindale is a patient concierge for San Diego plastic surgeon Dr. D. Glynn Bolitho. Dedicating more than 25 years in the cosmetic surgery industry, Ms. Martindale brings a wealth of knowledge to patients preparing for a consultation.

Ms. Martindale’s sage advice is to plan ahead.

Top plastic surgeons like Dr. Bolitho are going to be booked about four to six weeks in advance. So most of the time, a patient will not be able to walk in and request surgery the very next week,” Ms. Martindale said. “With that said, it’s never too soon to come in for a consultation.”

According to Ms. Martindale, some patients who want to learn more about liposuction or tummy tuck may think that they need to be at their ideal weight before their consultation. Ms. Martindale shared that potential patients do not need to wait.

“It’s always a great idea to come in for your consultation three or four months ahead of time so future plans can be made. Even though you may not be at your ideal weight, coming in and having your consultation is a great start,” Ms. Martindale said. “Many patients have also explained how the consultation served as a great incentive to help them achieve their weight loss goal.”

In other instances, achieving a certain weight loss for a particular procedure may not be necessary. Ms. Martindale shared an example such as for those wanting a neck lift or surgical facial rejuvenation procedure.

“If you’re within 10 to 15 pounds of your ideal weight, having a facelift may not make that much difference,” Ms. Martindale said.

Planning ahead is also needed if one is smoking traditional cigarettes or inhaling e-liquid nicotine while vaping.  Ms. Martindale wants potential patients to know that they need to stop smoking four weeks before and after surgery. And on a side note, Ms. Martindale happily reported how many of their patients never returned to their cigarette use.

Writing down questions ahead of time was also mentioned by Ms. Martindale.

“These questions can vary, but generally they range from recovery, when they can resume their exercise routines, and when they can fly,” she said.

For marathon runners readying for a future event, planning ahead on the opportune timeframe for surgery is important to address.

“Busy moms also need to coordinate their surgery time with their children’s schedules and possible childcare assistance,” Ms. Martindale said. “Another thing a patient will want to inquire is if their plastic surgeon will be out of town during the time they are interested in having surgery.”

In many respects, a plastic surgery consultation can help turn a possibility into a reality.

Fall into Beautiful Skin

Goodbye Summer. Hello Autumn. While many had the best of intentions to protect their skin during the summer months, they still may have had a little bit too much sun exposure.

With the season of fall officially here, there are ways to restore and rejuvenate the skin.

“I love to start with a series of chemical peels,” said aesthetic nurse specialist Elizabeth Reed, who works alongside San Diego plastic surgeon Dr. D. Glynn Bolitho. “I think that’s a great way to remove some of the dead skin that we’ve accumulated during the summer.”

Reed shared that typically skin care professionals don’t do chemical peels during the summer months because people retreat outdoors. Now is the perfect time to start that series.

According to Reed, a great way to see real improvements in one’s skin is by implementing a series of three chemical peels which are tailored to a person’s skin type.  These peels would be done every month.

“During this time, we’d also talk about a person’s skin care regimen and see what changes, if any, would be recommended,” she said. “And if someone is interested in Botox to address their fine lines and wrinkles, we would assess that to determine if they would be a good candidate. But really, starting with a series of chemical peels is a great way to fall into beauty.”

While people begin to change their skin care routine for the new season, Reed shared that there are three skin care staples that never changes. They are the following:

  • Vitamin C
  • Retin-A
  • Sunscreen

An application of Vitamin C in the morning and throughout the day helps hydration and photoaging protection from the sun.

“It also helps with pigmentation challenges such as dark spots, age spots or sun damage,” she said.

Reed’s second go to product is Retin-A which is a medical grade form of Vitamin A.

“Retin-A actually helps reduce some of the effects of sun damage and helps stimulate collagen,” she said. Reed added, “It will help with fine lines and wrinkles as well as exfoliating the skin to help rid it of brown spots.”

Retin-A comes in different grade levels. Reed likes to start patients off with a lower dose, and as one’s skin becomes accustomed to it, the dose level is increased.

The third item on the list is sunscreen which should be tailored to a person’s specific skin care type. A solar protection is imperative to help ward off future damage and help maintain the skin care treatments a person is receiving.

“I usually pick something that has a high percentage of zinc, because zinc oxide is our physical sunscreen,” she said.

Changing the face of science

Science continues to redefine itself in the area of facial rejuvenation. With an eye to the future, a recent article published in JAMA Facial Plastic Surgery has paved the way to new ideas and possibilities. And in the world of science, it’s all about new concepts.

While facial surgery such as facelifts, cosmetic injections, and even fat grafting have filled a vital niche for many patients opting for rejuvenation, researchers are testing to see if the human body can somehow perhaps be encouraged to stimulate facial tissue regrowth.

This area of science is called regenerative medicine.

In Vocativ, reporter Alexandra Ossola covers this recent article which was authored by Matthew Q. Miller, MD. Miller is a surgeon specializing in the head and neck regions from the University of Virginia Health System.

Ossola writes, “Though scientists’ interest in regenerative medicine has grown steadily since the early 2000s, interest in facial surgeries in particular isn’t reflected in the literature until 2010. Miller believes there are two reasons for this. Generally, facial surgeries have good outcomes—they heal quickly and have a low rate of infection, so there’s no need for dramatic improvement.”

Miller also shared that there is not “margin for error” in operative facial surgery.

“Even small imperfections can really stand out, as people’s eyes always focus on the faces of others in day-to-day interactions. Because of this, new techniques are a little slower to gain widespread acceptance among surgeons,” Miller said.

Also interviewed in the article was Richard D’ Amico who serves as the chair of the Regenerative Medicine Task Force which is a branch of the American Society of Plastic Surgeons.

D’Amico notes how hyaluronic acid fillers support facial structures. Fillers such as these are noted to add a more youthful appearance when volume or elasticity has dissipated due to age or premature aging. While injections of this type such as Juvederm offer a great outcome, the results are temporary lasting up to a couple of years.

While patient dependent, fat grafting is generally utilized during a facial surgery to add volume. Following liposuction of a small area such as the thighs, abdomen, or hips, the fat is then filtered via a centrifuge before the injections occur in the rejuvenation procedure

In the article, D’Amico described this process as labor intensive thus requiring it to be more expensive for the patient.

“If regenerative medicine works the way researchers think it could, patients could just get a quick injection, plus some localized anesthesia, to repair their facial tissues,” Miller writes. “And while it might not be able to repair tissues in a “Star Trek-like fashion” (that is, with a device that can heal wounds almost instantly), Miller and his co-authors write, these regenerative techniques could usher in a new age of plastic surgery for the face.”

The three levels scientists are currently focused on include the following:

  • Stem Cells
  • Platelet Rich Plasma (PRP)
  • Protein Matrix

While more research is underway and these above scientific avenues still require more answers, it’s interesting to know how science continues to play a role in facial rejuvenation in one form or another.
